However, around 1980 pessimists predicted the premature demise of cardiovascular nuclear imaging with the introduction of digital subtraction angiography and nuclear magnetic resonance imaging.

In recent years there has been an increasing interest in quantitative analysis of coronary cineangiograms and already for a longer time of left ventricular cin­ eangiograms. The needfor quantitationofcoronary arterialdimensions has been stimulated by the introduction ofnew therapeutic procedures in the catheteriza­ tionlaboratory, suchas the balloon dilatationtechnique (PTCA) and thromboly­ tic therapy, by the need to study the vasoactive responses of pharmaceutical agents, and also by the desire to study the progressive nature ofcoronary artery disease with the ultimate goal to find ways to bring a halt to the progression of coronary atherosclerosis or even achieve regression of the disease. Parallel with these clinical developments, rapid technical developments in computerarchitect­ ures and semiconductor memories have made it possible to digitize and store cineframesor selected portions thereof in image processors and to analyze these pictorial data quantitatively at affordable prices. More than 15 years of research have been directed by various groups towards the semi- or fully-automated delineation of the left ventricular boundaries on a frame-to-frame basis. Yet not a single system with fully-automated capability is commercially available. In the mean time many different left ventricular wall motion models have been developed, again with little consensuson which model is to be preferred as no golden standard exists.

In November 1986, I was invited to attend a symposium held in Barcelona on Diseases of the Pericardium. The course was directed by Dr. J. Soler-Soler, director of Cardiology at Hospital General Vall d'Hebron in Barcelona. During my brief but delightful visit to this institution, my appreciation of the depth and breadth of study into pericardial diseases, carried out by Dr. Soler and his group, grew into the conviction that these clinical investigators have accumulated a wealth of information concerning pericardial diseases, and that investigators and clinicians practicing in English speaking countries would greatly profit from ready access to the results of the clinical investiga­ tions into pericardial disease carried out in Barcelona. The proceedings of the Barcelona conference were published in a beauti­ fully executed volume in the Spanish language edited by Dr. Soler and pro­ duced by Ediciones Doyma. Because I believe that this work should be brought to the attention of the English speaking scientific and clinical com­ munities, I encouraged Dr. Soler to have the book translated into English. I knew that this task could be accomplished and that the book would be trans­ lated into good English without change of its content. My confidence was based upon a translation of my own book, The Pericardium, into Spanish undertaken by Dr. Permanyer, who is a contributor and co-editor of the pre­ sent volume.

In industrialized countries, ischemic heart disease is by far the most common organ-specific cause of death. The failure of a heart with an ischemic area may be due to the amount and location of contractile tissue becoming paralyzed or even necrotic, or to arrhythmias provoked by the ischemic condition, or by a combination of both factors.

In recent years considerable progress and new developments in diagnostic and interventional cardiology have been observed, such as balloon angioplasty of coronary artery stenoses, reperfusion techniques in acute myocardial infarction, new pacing, and cardioversion-defibrillation techniques in ventricular tachyar rhythmias.

The book is so designed that clinicians may be able to read the relevant chapters in the second part of the book in isolation, using the basic science aspects contained in the first part of the book as reference chapters.

Although clinicians have recognized the importance of inflammatory mediators in the pathogenesis of heart disease for well over 200 years, it has taken nearly as many years for clinicians and scientists to focus on the basic biological mechanisms by which inflammatory mediators contribute to the pathogenesis of cardiac disease states.
